I found it rewarding and enjoyed assisting patients with the safe discharges from the hospital. In particular, I enjoyed figuring out the reasons and barriers which prompted each patient to keep coming back to the ED or hospital, and then resolve them. I believe that I will be able to continue providing excellent services for the Population Health here at the Resource Center.
