Development of NC Medicaid Network

Duke University Health System and Community Care of North Carolina develop the Durham Community Health Network. Network re-named Northern Piedmont Community Care in 2005.

2003

Launch of Duke Employee Care Management Program

Duke launches Duke Prospective Health, a care management program for Duke employees designed to improve wellness and productivity through management of health risks and chronic conditions. Program re-named DukeWELL™ in 2011.

2006

Responsibility for Durham's Under/Uninsured

Duke Health assumes fiscal responsibility for LATCH, a program from Health Communities Access Program (HCAP) and US Health Resources and Services Administration (HRSA) serving the under/uninsured in Durham.

2013

First Commercial Narrow Network Partnership

Duke Health partners with Coventry to launch a commercial narrow network in anticipation of the 2014 North Carolina Health Insurance Exchange rollout. Duke Health partners with Cigna (Collaborative Accountable Care), Blue Cross Blue Shield (Blue Local) and Aetna (Aetna Whole Health) in later years. All partnerships include access to DukeWELL care management services.

Duke Health develops Duke Connected Care, a clinically integrated network consisting of Duke Health and independent providers in the Greater Triangle who are committed to delivering high-quality, cost-conscious care.

2014

Participation in Medicare Shared Savings Program

Duke Connected Care begins 3-year agreement with Medicare as a Track 1 (non risk-bearing) Medicare Shared Savings Program Accountable Care Organization (MSSP ACO). Duke Connected Care renews agreement for four more years starting in 2017. New agreement includes move to Track 3 (risk-bearing) ACO starting in 2018.

First Medicare Advantage Partnerships

Duke Health partners with Aetna and Humana on products that align cost and quality incentives in care delivery for Medicare Advantage patients. Products include care management services by DukeWELL. Duke Health partners with United HealthCare on Medicare Advantage product in 2017.

Duke Health centralizes and unifies outpatient care management functions provided through DukeWELL, NPCC, and LATCH through launch of new entity: Duke Population Health Management Office (PHMO).

Duke Connected Care Tops All NC ACOs

Duke Connected Care exceeds evidence-based quality standards while outperforming all NC Medicare ACOs in dollars saved ($21 million) and beneficiaries served (52,000).

2018

Duke Connected Care Shifts to Risk-Bearing Payment Arrangement

Duke Connected Care shifts to advanced alternative payment model (aAPM) under Medicare Shared Savings Program (MSSP) that includes financial risk tied to outcomes of assigned patients.

Duke Connected Care Launches Medicaid Tier of Clinically Integrated Network

New tier will provide efficient mechanism for practices to contract with prepaid health plans and meet the analytics, reporting, and care management capabilities to deliver the best outcomes possible for patients under Medicaid Managed Care.

The Duke Population Health Management Office signed on with NC Medicaid to deliver integrated health care for individuals with behavioral health needs and intellectual/developmental disabilities.
