Our History & Growth
Duke Connected Care was established in 2014 as part of Duke Health’s commitment to advancing value-based care. Since its inception, DCC has grown into a robust, clinically integrated network that serves a vast and diverse patient population. With an emphasis on care coordination and population health management, DCC now supports more than half a million lives across 8 different counties in the Triangle region, ensuring they receive high-quality, cost-effective care.
Today, the network includes more than 2,500 primary care providers and 4,000 specialists, working in collaboration with 4 hospitals and 50+ skilled nursing facilities.
How It Works
We get to know your practice. Your Physician Liaison will be your single point of contact and will be available to assist you at any time. They will visit your clinic regularly and spend time with your staff.
We handle your reporting. We will increase your chances of success by reporting to Medicare on your behalf, giving you the tools to improve performance, and you will be evaluated as part of DCC’s high-quality network.
We help take care of your patients. You will have access to one-on-one support from a dynamic team of DukeWELL care managers, all at low or no cost to you or the patient. As a DCC practice, you will be able to refer patients to DukeWELL for patient support services. Learn more at DukeWELL.org.
