LATCH Participates in Community Health Surveys

Several members of the Local Access to Coordinated Health Care (LATCH) care management team participated in Durham County’s Community Health Assessment Surveys during October 13 and 14, 2016.

LATCH’s bilingual staff, Liliana Marin, Maria Elisa Padilla, Alma Ramirez, Yholima Vargas-Pedroza, Maricela Caceres and Katherine Ruiz traveled door-to-door to randomly selected addresses throughout the county from 9:00 a.m. to 5:00 p.m. to conduct the Community Health Opinion Survey (CHOS) in an approximately 20-minute long Spanish-language interview.

LATCH Provides Patient Navigation and Interpretation at 2016 Men’s Health Initiative

Local Access to Coordinated Healthcare (LATCH) representatives participated in the 2016 edition of the Men’s Health Initiative. Community health worker Alma Rodriguez provided interpretation on Saturday, September 17 at Lincoln Community Health Center. Social worker Nick Conde-Dudding, disability case worker Tashira McGill, nurse case manager Bill Smith and health education specialist Natacha Rubio provided patient navigation, interpretation and orientation services on Sunday, September 18 at Duke South.

LATCH at El Centro Hispano's Celebration of Health

Local Access to Coordinated Health Care (LATCH) staff participated in the 2016 edition of El Centro Hispano’s Celebration of Health on November 5th, 2016. LATCH representatives provided information to uninsured and insured patients about primary care options and other resources available to the English and Spanish-speaking community.

The event took place at El Centro Hispano’s new location in the Lakewood shopping center in Durham. Participants in the health fair received health screenings, cooking classes and many more services.
