We're There For Your Patients—When You Can't Be
PHMO Patient Care Support aims to improve health outcomes by ensuring patients are able to follow their treatment plans and access the right care at the right time. We provide two types of services:
- DukeWELL™ Ambulatory Care Support helps eligible, high-risk patients access the right care, transition across care setttings, and adhere to treatment plans.
- The Duke Health Resource Center provides inpatient and discharge care support to patients who are admitted into any of Duke Health's three hospitals.
