Support For Patients During & After Their Hospital Stay
The Duke Health Resource Center supports case management activities for all patients who are admitted into any of Duke Health’s three hospitals with thorough post-discharge follow-ups, including:
- Disease-specific follow-up
- Follow-up appointment scheduling
- Inbound and after-hours call support for:
- Discharged patients
- Skilled nursing facilities
- Referral to skilled nursing facilities, including those in the HOPE Skilled Nursing Facility Collaborative
