Two Networks, One Goal

We administer two networks that share a common goal: to provide a seamless, high-quality healthcare experience for patients and their loved ones. Each network has access to the full range of PHMO services.

  • Duke Connected Care (DCC) is a physician-led, clinically integrated network and accountable care organization that qualifies as an advanced alternative payment model under MACRA. DCC is comprised of Duke Health's primary and specialty care providers and select independent practices. Participants have access to Medicare and commercial value-based contracts.
  • Community Care of North Carolina (CCNC) is a North Carolina Medicaid networks comprised of practices in Durham, Franklin, Granville, Person, Vance and Warren Counties who treat Carolina Access II Medicaid beneficiaries.
