The Duke HOPE (Health Optimization Program for Elders) Program is an initiative led by Duke Geriatrics to assist older adults transitioning from an acute care setting (hospital) to a skilled nursing facility (SNF) for rehabilitation and nursing care. HOPE Leadership has created this booklet to help patients and caregivers make informed decisions about the transition. Some questions addressed include:
- What should I consider when choosing a SNF?
- Does insurance cover SNF services?
- What should I expect at the SNF?
In addition to the information contained in this booklet, HOPE Consultations are available at Duke University Hospital and Duke Regional Hospital. Through this program, each patient’s specific needs are identified and the transition of care from hospital to SNF is reviewed by a geriatric provider to improve continuity of care.
