I’m a registered nurse. My clinical experience is in orthopedics and neurosurgery. I’ve cared for patients recovering from musculoskeletal injuries, joint replacements, spinal surgeries, and complex neurological conditions, focusing on post-operative care and patient education. In my previous role, I worked as a triage nurse across multiple ambulatory care specialties, such as endocrinology, urology, hematology, sickle cell, pain, infectious disease, geriatrics, keto medicine & weight loss, and primary care.

Lexi Galford, BSN, RN, PCCN

I began my nursing career with Duke in 2020, working through inpatient cardiology stepdown, cardiac procedures, outpatient hospice, and community health roles. I love meeting patients and their families and believe that modern medicine is about walking with people through each season that life brings. I currently live in Raleigh with my husband and fluffy golden retriever.

Meg Hatley, MSW, LCSW

I'm a licensed clinical social worker with over 15 years experience in care management and behavioral health treatment. I use a client-centered approach to support individuals in clarifying their values and aligning their health goals with what matters most in their lives. I work to help clients identify barriers to wellness and to develop practical, sustainable strategies for change. I love having the opportunity to empower people to live healthier, more meaningful lives.

Ana Valdes, BSN, RN

I am a registered nurse with extensive experience in home health and health care operations, including clinical leadership, team management, and performance improvement. My background includes overseeing daily operations, supporting multidisciplinary teams, and driving quality, compliance, and patient-centered outcomes. I bring a collaborative leadership style, strong organizational skills, and a commitment to creating efficient workflows that support both patients and staff.

Sara Miller, BSN, RN

I have spent six years at Duke Health as a bedside nurse serving over twenty units at all three hospitals. Providing care for patients and their families during such a stressful time is what led me to this care manager position with DukeWELL. I want to empower people to better manage their health at home to try and avoid such difficult times.

Crystal Bennett, MSN-Ed, RN, CCM

I have over eighteen years of nursing experience, including nine years in case management within acute care, particularly in trauma and cardiovascular thoracic surgery. My background also includes work in orthopedics, long-term care, ICU, and academia. However, I have found care management to be the most rewarding role in my career.

Sherri Harris, MSW, LCSW

I am a licensed clinical social worker with a background in mental health. More recently, I’ve worked in medical case management and supported patients with complex medical and psychosocial needs at an area hospital. Throughout my career, I’ve valued the connections I’ve made with patients and families. In my role with DukeWELL, I’m excited to be a part of supporting positive health outcomes by connecting patients to follow-up care and resources.
