Programs That Make A Difference

We participate in grant-funded and charity care initiatives aimed at improving the health of underserved and vulnerable populations.

• Duke Health HOPE Skilled Nursing Facility Collaborative is comprised of Greater Triangle skilled nursing facilities that are committed to collaborating with Duke Health around care transitions with the goal of improving outcomes for our shared patients.
• Local Access To Community Healthcare (LATCH) is a program that provides the under- and uninsured in Durham County with FREE access to DukeWELL Services and improves access to disability benefits using SSI/SSDI Outreach, Access and Recovery (SOAR), a national program designed to increase access to the disability income benefit administered by the Social Security Administration (SSA) for eligible patients.
