Dr. Susan Spratt, PHMO Senior Medical Director, continues to lead the national discussion in care gaps for people living with diabetes. In the Fall, she spoke at a value-based medicine population health event held by the American Journal of Managed Care in partnership with Duke Health, which featured faculty from Duke University Medical Center and Duke University School of Medicine discussing the origins of these gaps and actionable strategies for addressing them, like best practice advisories in Epic and SDOH support programs.
Susan also shared how PHMO is focusing on whole-person care by screening for health-related social needs and linking patients to programs in the community that can help, like the Eat Well produce prescription program that gives eligible participants money to buy fruits and vegetables each month.
