
Highlighting another way that PHMO's DukeWELL Care Management team connects patients with the right care, in the right place, at the right time, INDY Week recently reported on a new collaboration providing mobile harm reduction treatment to neighbors in Durham.
For months, DukeWELL's Mahogany Woods, care coordinator on the AMH+ Care Management team, has joined a coalition of community partners—including Durham Technical Community College and the North Carolina Harm Reduction Coalition—for Project MAPS (Mobile Addiction Treatment & Primary Care Services) treating afflictions like HIV and Hepatitis C throughout the community. The mobile unit provides care right in the community where it's needed most.
Project MAPS and other Durham wellness initiatives will benefit from the distribution of the recently awarded NC opioid settlement to intervene in the cycles of addiction and improve health outcomes for Durham residents.
